The New York State Department of Health has released information as well as a FAQ for how it will handle applications for OBCs. Submissions will not be accepted until January 15, 2020, the effective date of the law. Please read this information, especially the FAQ, carefully.
New York CITY and all of its boroughs will have a different procedure. That application information is not yet available online, but will be posted shortly on the NYC Health Department webpage. It will not take applications before January 15.
Please Note: The STATE forms that are online now are NOT the forms that will be used. Relevant forms will NOT be available until the 15th when the law takes effect.
If you would like to receive your OBC in the most expedient manner you should apply online NOT in person or by mail.
